To use the Public Access Portal

  • Allow this page to generate pop-ups (do not block pop-ups).
  • If not already installed, install Adobe Acrobat Reader (used to view the Official Immunization Record). Download for free at http://get.adobe.com/reader/
  • Make sure to enter the correct first name, last name, date-of-birth, and phone number/email at the time of the requested immunization(s).  For example, if requesting a shot record and the last immunization occurred in April of 2020, use the phone number/email that was being utilized in April of 2020 for that immunization.
  • A maximum number of 3 attempts to search for a patient record is allowed.  If the maximum number of search attempts is reached, your account will be locked (IP address). Please wait 15 minutes and try again.
