ATTENTION
As the Oklahoma Immunization Service grows closer to achieving electronic exchange of records,
new changes are being made to OSIIS. This includes a new
Authorized Site Agreement,
which is available at the OSIIS web page.
This Agreement:

Reminds Authorized Sites and users that OSIIS contains protected health information subject to the legal requirements of the Health Insurance Portability and Accountability Act of 1996 (“HIPAA”);

Emphasizes the continuing obligation of Authorized Sites and their users to obtain authorizations for the use and disclosure of information in OSIIS where such use or disclosure is not otherwise permitted under HIPAA or other applicable federal or state law;

Requires Authorized Sites and users to retain documentation of authorizations and OSIIS User Agreements for at least six (6) years in accordance with HIPAA document retention standards; and

Reminds Authorized Sites and users that their use of OSIIS must comply with the Authorized Site Agreement, user agreements, the Confidentiality and Security Policy for OSIIS, and applicable federal and state law, rules, and regulations and that failure to do so may result in termination of the Agreement and access to OSIIS.
